Overview
The purpose of this workshop is to think about the movement of the Oil SSTS from manufacture into in-service, and in particular what the operator needs to know about the Oil SSTS to ensure its safe operation.
Task
Response to Manufacturing Tests and Trials
It has been discovered that the Oil SSTS control system fails with a higher than expected frequency, which has resulted in an intolerable safety risk that remotely operable valves could open inadvertently. It is not possible to change the design, so consider what else could be done to reduce the residual risk to tolerable levels.
Review your the hazard log for your preferred option and make any necessary changes to the SCR below.
Operator Information
Review your Oil SSTS Safety Case and the hazard log for your preferred option and update Section 6.0 in the SCR below (this is the Main Gate SCR you previously populated), identifying:
- The main areas of residual risk.
- Any limitation on operation that the operator should be made aware of.
- What PPE is required.
- What other activities/evidence is required to ensure the safe operation of the Oil SSTS
Response to Main Gate Audit
Review your auditors comments and update Section 7.0 in the SCR below (this is the Main Gate SCR you previously populated), to identify the key comments and your responses.
