Overview

This page includes the SMP04 checklist, which will be used to complete preliminary hazard assessment of the proposed Oil SSTS. The output of which will be used to support the development of the URD and subsequently the SRD as part of the wider integrated set of safety activities required to support development of an acceptably safe system into service.

As can be usual at this stage, there is little information to base the assessment upon, however your team will complete the assessment against a functional block diagram of the Oil SSTS.

Task

Oil SSTS System Block Diagram

Review the information provided in the Scenario and create a system block diagram for the proposed Oil SSTS using the box below. [Hint, you can make/delete blocks and each block can be connected with a label]. This system diagram will help visualise the proposed system and support hazard identification.

Made with Padlet

[Hint, an example of a block diagram and how yours might look is provided here]

Hazard Identification

Using the General Checklist found in ASEMS SMP04, your needs to identify hazards associated with the proposed Oil SSTS as detailed in your block diagram.

Hazard Log

wdt_ID ID System Function / Component Hazard Title / Description Prelim - Accident Sequence Required Controls / Mitigation
399 1 Well Head Oil Transfer Overpressure: Over pressure of the well-head Oil spill due to structure failure caused by Overpressure causes in well head PRV. Pressure sensing. Over pressure shutoff manual/automatic
404 2 SSTS Isolating Valves Flammable substances; e.g. solid, liquid or gaseous Overpressure of pipe work, valve failure, control system failure, leaking flammable substances , risk of fire., environmental concerns. Periodic inspection and maintenance. PRV, Pressure sensors. Automatic shutdown. Secondary isolations, fire detection and suppression
405 3 Transfer Pump Mechanical Failure Failure cause high temperature which leads to fire risk. Inspection and maintenance. FD&F systems.
406 4 Oil Separator Fire and Explosion Hazard Flamable liquids and vapours come into contact with ignition source resulting gin fire and explosion. Maintenance and inspection, PRV, Pressure sensors, Bunded tanks. FD&F system
407 5 Oil Separator Control System Hazard Instrumentation system, control logic or HMI error leading to hazardous substances leaks, Fire Same as ID4
408 6 Burn Off Gas Fire & Explosion Hazard Leaking gas from pipework, or accumulation of unburned gasses, comes into contact with an ignition source resulting in uncontrolled fire or explosion Maintenance and inspection, metering, pressure sensors, PRV, FD&F,
409 7 Flame Tower Mechanical/Structural Failure Impact damage or corrosion causes and structural failure resulting in crushing, fatality, sympathetic damage to other systems Maintenance, Inspection, PPE, controlled access zones.
410 8 Metering failure Oil Spill Inaccurate measuring of transferred liquid causing spillage and environmental contamination. Maintenance, Inspection, Calibration, Pressure sensors, Remote cut-off.
411 9 Storage Spillage Spillage due to over filling, leaks or structural damage PRV, Maintenance, Inspection, Sensors, Remote cut-off, Suitable materials
412 10 Flame Tower, Transfer System, Separator & Storage Shock and Vibration Excessive Shock and Vibration results in failure of critical system components, leading to leak of flammable gasses and liquids.
