Workshop Objective

The Functional Hazard Analysis (FHA) has identified concerns regarding the consequences relating to fuel containment and an Event Tree Analysis (ETA) is required to understand the associated consequences and overall impact to the aircraft.

The objective of the workshop is to consider the sequence of events that would follow a fuel containment failure within the pod and explore/determine the various consequences that might arise.

  1.  Select an ETA leader and recorder. The leader will facilitate the ETA workshop under the guidance of the Tutor. The Recorder will draw up the Event Tree.
  2.  Carry out ETA for the refuelling pod.
  3. Guidance:
    • Discuss & Identify the sequence of events step by step.
    • Discuss & Identify safeguards that could influence the accident sequence.
    • Record events & safeguards as an aside before trying to shape the event tree.
    • Determine the consequences that could result.
