Overview

This workshop aims to increase your understanding of ASPIRE Guidance and Risk Assessments, using the BowTies to identify which functional Threats and Hazards/Accident Sequences are impacted by the introduction of the equipment (Wing Pod) onto the aircraft. Additional detail is provided in ASPIRE tool 17.A.1.

Workshop Objective

Once the associated Functional Threats and Hazard/Accident Sequences have been identified, for each hazard, develop the example BowTies to illustrate the equipment contribution to RtL.

For each Hazard, identify:

  • The Equipment Functional Threat (left of BowTie);
  • Equipment Controls using ASPIRE Taxonomy, including:
    • Equipment Prevention Controls; and
    • Equipment Recovery Controls.
  • Non-Equipment Controls that needs communicating to the ADH to reduce the risk further.

For Each control, assess the Effectiveness of the control using the ASPIRE guidance provided below.

Key for Padlet

PC – Preventative Control

RC – Recovery Control

EFT – Equipment Functional Threat

Below is an example of how the BOW-TIE should look upon completion of the Padlet. For further help a PDF has been linked here.
